  • General job duties include assessing patients for discharge planning and psychosocial needs, working with an interdisciplinary team to develop care plans, and providing overall care coordination for a team of patients.
    • Job Requirements
    • Must be proficient in care coordination, discharge planning, and assessment of psychosocial needs/support.
    • Must have previous experience providing care coordination/discharge planning.
    • Current IL RN license required.
      • Schedule Information
      • 8 hour shifts - M-F with rotating weekends.
      • Call daily until 6pm.
        • Unit Specific Information
        • Floating to other units/departments may occur, covering approximately 15-20 patients per day.
          • Additional Details
          • Does candidate require any time off during initial assignment? If yes, indicate dates in comments section.
          • Is the candidate a 1099?
          • Must be proficient in care coordination, discharge planning, and assessment of psychosocial needs/support.
          • Must have previous experience providing care coordination/discharge planning.
          • Case Management certification preferred.
          • BLS - American Heart required.
          • Need to use a personal cell phone for communication.
          • BSN required.
          • Candidate must understand OSF is a Drug Free Campus and they must produce a negative drug screen for all panels.
