  • General job duties include assessing patients for discharge planning and psychosocial needs.
  • Working with an interdisciplinary team to develop a plan of care for patient transition.
  • Providing overall care coordination for a team of patients.
    • Job Requirements
    • Must be proficient in care coordination, discharge planning, and assessment of psychosocial needs/support.
    • Must have previous experience providing care coordination/discharge planning.
    • Current IL RN license required.
    • Case Management certification preferred, but not required.
      • Schedule Information
      • 8 hour shifts - M-F with rotating weekends.
      • Call daily until 6pm.
      • Floating to other units/departments may be required.
        • Unit Specific Information
        • Covering approximately 15-20 patients per day.
        • Nurse to patient ratio: n/a.
        • Business Casual dress code with closed toe and closed back comfortable walking shoes.

          • BLS - American Heart certification required.
